- Resume/Cover Letter with a minimum of 3 supporting professional references (Teacher/Support Staff)
- Copy of Teaching Certificate (Teacher)
- Copy of TQS (Teacher)
- Verification of Employment (Teacher)
- Original Criminal Record Check (dated within one year) (Teacher/Support Staff)
- Original Child Welfare Check (dated within one year) (Teacher/Support Staff)
Submit the above documents. If you are approved, you will be asked to submit the documents below:
- Direct Deposit Form
- 2012 TD1 Form
- 2012 TD1AB Form
- Employee Code of Ethics
- Technology Acceptable Use Agreement Form
- Technology Acceptable Use Policy
- ASEBP - Benefit Plan Letter (OPTIONAL)
- ASEBP - General Plan Provision Summary
- ASEBP - Declaration of Eligibility (part of Medical Evidence)
- ASEBP - Medical Evidence of Good Health Form - All substitutes are eligible to participate after working 5 days. If you have any questions, please contact ASEBP directly at 1-877-431-4786.
NEW TEACHERS that recently graduated (Alberta only)
- Get a letter from the Registrar citing that you have met all the application requirements for your Teacher Certification.
- Get an Offer of Employment from PHRD stating that we have offered you employment.
- Get a letter from the Dean of Education stating that you have met all requirements for certification.
- Combination of all three letters allows you to teach 90 days while waiting for the recommendation from the Dean to issue your interim certificate.
